Use this secure link to upload medical records (labs, imaging reports, PDFs, discharge summaries, clinic notes). Do not upload urgent/emergency concerns.
Important:
Open Secure Upload Portal
- Only upload records for the purpose of clinical review/telemedicine coordination.
- Include the patient’s full name and date of birth in the file name (or inside the document).
- Avoid sending passwords in the same message as the files.
What to upload
- Lab results (PDF).

- Endoscopy/pathology reports (PDF).
- Hospital discharge summaries (PDF).
- Prior clinic letters/consult notes (PDF).
